St. Patrick's is the only Elder Care Facility in the Greater Sacramento area to offer:

Four Levels of Care permits the isolation of needs unique to a single Client and reflects a Monthly Care Rate only appropriate for that individual's level of need. Levels of care, and the corresponding rate, may change as the Client demonstrates the need for closer supervision or greater personal assistance. A description of each Level is offered in this Web Site to assist interested parties in their understanding of Level Care Management. RESPITE CARE - Day, Night Short Stay
LEVEL ONE CARE - Typically, a client assessed to have characteristics associated with early dementia require the basic level of supervision of Activities of Daily Living - ADL delivered to our Level One Clients in Residence.
LEVEL TWO CARE - Level Two Care accommodates the increasing need for 1) stand-by supervision in Activities of Daily Living - ADL, and 2) hands-on care by trained staff, in support of weakening continence, the beginnings of wandering and growing confusion.
LEVEL THREE CARE - Level Three Care accommodates the greater need for 1) stand-by supervision in Activities of Daily Living - ADL, and 2) hands-on care by trained staff. Level Three Clients can demonstrate total incontinence, greater confusion, wandering, displays of inappropriate social behavior, aggressiveness and/or resistence to hands-on care requiring frequent increases in the number of staff assigned to a Level Three Client in Residence.
LEVEL FOUR CARE - Level Four Care accommodates the unquestionable need for 1) continual supervision of Activities of Daily Living - ADL, and 2) total hands-on care by trained staff. Level Four Clients can demonstrate total incontinence, constant confusion, wandering, displays of inappropriate social behavior, aggressiveness and/or resistance to hands-on care, bed-side care, or transferring which result in the permanent increase in the number of staff assigned to a Level Four Client in Residence. Aberrant toileting habits can require frequent, and additional housekeeping and laundry.
HOSPICE CARE - The highest level of care is reached when clients, loved ones and staff come together to share the last steps in this long journey. Level Five Care has been created to provide comfort measures, staff attentions and vigils of family and friends. Every effort has been taken to preserve the privacy and dignity of all concerned as a unique life comes to its close.
A client's appropriate level of care is determined by the Administrators of St. Patrick's through their careful examination of the patient¹s health records, professional diagnosis, medication history, attending physician¹s statement and interviews with persons associated with the patient.
A regular census of Levels of Care makes curtain that St. Patrick's higher standard of supervision is maintained with the necessary trained staff and supervisors.
Our Administrator and Medical Technicians monitor all medications, therapies, and transfers to scheduled doctor¹s appointments.
Our Administrators will do everything possible to fill requests for private or shared room accommodations - based on availability at the time of admission. St. Patrick¹s maintains a waiting list to fulfill requests as accommodations become available.
